Executive Summary

For seniors using five or more medications, the risk of adverse drug reactions (ADRs) is nearly 50% – Almost one out of every two people! It is estimated that up to 30% of hospital admissions of the elderly involve medication-related ADRs.

Senior Health Services (SHS) is a home-visitation service that helps older individuals oversee their pharmaceutical care. Our intent is to grow this sorely needed monitoring program into a National Service Corps designed to strengthen ‘preventive’ geriatric healthcare, thereby saving thousands of lives and millions, perhaps billions of dollars across the country.

Rarely these days do the elderly have a single prescribing physician and a single pharmacist to coordinate an individual’s overall pharmaceutical care. We at the Polypharmacy Screen, Senior Health Services (SHS)*, will consult in that critical role — to assist the patient, the home caregiver, and even the prescribing physician with a ‘safety-review’ of the patient’s total ‘polypharmaceutical’ treatment profile.
